During our time together, we will discuss: Your health goals Your biggest challenges Your individualized 3-month program This appointment can be in-person at our Taylors, SC location, or via Zoom: If you are not familiar with Zoom, please go to https://zoom.us to download it on your computer. Important Details: A detailed (4-page) Initial Intake Form, 3-day Food Journal, and Parasite Questionnaire will be sent after you book your appointment. ALL forms need to be completed fourteen days before your appointment. Please take this time into consideration when booking your appointment. The extensive time I take to review and do research allows me to create an individual plan just for you. The Initial Holistic Health Consultation appointment is 2 hours long, and is an in-person appointment. Book your appointment at least two weeks out from when you think you will complete your detailed intake form. An individualized program will be created which may require weekly appointments. During your Initial Consultation, package options will be explained. I offer 3 month Individual Wellness Plan packages based on YOUR health concerns.
